The default grade for an item is setup in OPM Quality Manager. The grades used will be given by the company for whom implementation is being done.
Status of an item is the initial status when an item is received as Raw Material or when a Finished Good is produced.Eg. of Item Status are Unapproved,Approved,On Hold etc. If a Finished Good item has a status of Unapproved this means that quality checks are to be performed before it is to be released into the market.

Just to add to Mathew's response; the default Grade and Status are both defined in the OPM Inventory module at the item master level.
Specific grades are then assigned to inventory Lots/sub-lots.

They are both user definable; ie you can detremine what you want to call them and how many of them their are.
The purpose of status is to detremine whether the inventory should be considered eligible for planning, use on batches, or shipping. As an example we have the following statuses:

A: Approved (can be used for all purposes)
Q: Quarantine (as yet untested but can be considered for planning purposes)
QR: Quarantine Restricted (not yet fully tested but approved for use in planning and consumption on batches)
QRS: Quarantine Restricted Shipping (can be used for all purposes but has not yet been fully tested - we sometimes use this to ship material to associate companies for further packing/processing)

Grade can be used to give another level of control around what can be shipped to specific customers (ie a grade can be assigned to a customer)
It can also be used to provide visibility of production or shipping restrictions.
